Medication and Immunization History
Surescripts fill history and the state registry, pulled before the visit.
Before the visit, the Care Sync agent pulls what pharmacies actually dispensed from Surescripts and every recorded shot and what is due next from the state immunization registry, and files both to the chart. The scribe writes against that history, so a lapsed refill or an overdue booster is on the page, not a question for later. Today’s shot is reported back to the registry the same way.

05The prescription
E-Prescribing
Written in the chart, sent over Surescripts to the pharmacy the patient chose.
The prescription is written inside the chart and sent electronically over the Surescripts network to the patient’s pharmacy. Medication history and plan coverage are on screen as you write, so the formulary surprise happens here, not at the counter. Renewal requests from the pharmacy come back to the same chart and are answered without re-entering anything.
06Controlled substances
Controlled Substance E-Prescribing
Schedule II to V, signed with two factors by the prescriber alone.
A controlled substance takes more than a click, and the chart enforces it. The prescriber is identity-proofed, holds a DEA registration and is enrolled in two-factor authentication. Each prescription is signed with a PIN plus a one-time code from the prescriber’s own phone. Those credentials cannot be delegated, so no one else can sign on a prescriber’s behalf.
